You can use this form to submit a complaint to us about how we have handled your personal data or your data rights.
By 'personal data' we mean information that contains anything that can identify you or another living individual.
We take our obligations under the UK-GDPR and DPA 2018 very seriously and we welcome the opportunity to make things right, so please contact us on this form in the first instance.
It is not mandatory that you use this form to submit your data protection complaint, but it does help us to gather the information we need to help deal with your complaint quickly. Please do not use this form for complaints that are not directly related directly to data protection. Instead, please review our general complaints policy and procedure.
This form will collect your personal data, such as your name and contact details as you enter them. Your personal data will be used to manage your complaint, and will only be shared with third parties where necessary to investigate your complaint, or where we have a legal or similar reason for doing so.

You will receive an automated email to let you know that we have received your complaint.
The Information Governance team will review your complaint and provide a response within 30 days. This is usually via email unless you request otherwise.
We may also ask you for further verification or information to help us investigate the complaint.
We will keep you informed during the process and of any delays.
