We will select one winner whose illustration will be on the next National Paediatric Diabetes Audit (NPDA) report cover and who gets a £50 gift card!
This competition is open to all children and young people (up to 25 years old) with diabetes, receiving care from a paediatric diabetes unit.
This form should be completed by the parent or carer of a child or young person, or by a young person who is 18 years or older.
